Today, I would like to review a case of bulbous nose, long nose, deviated nose, and columella correction.

First, let's analyze the issues through the patient's pre-operative photos.
Pre-operative Analysis

The patient generally had a low nasal bridge,
the nasal tip looked round, and the alar base was drooping.
Also, the nasal bridge appeared deviated.

The patient wanted a natural, nearly straight side profile,
along with improvements to the blunt nasal tip, alar base, sunken glabella, and deviated nose.
Diagnosis and Surgical Plan

Since the patient had a small nasal septum, we decided to build the strut using septal cartilage and donated rib cartilage,
correct the deviated nose, refine the nasal tip using ear cartilage for a slimmer look,
and use a silicone implant for a straight-line feel on the nasal bridge.
1 Week Post-op

There was not much bleeding during surgery, and hemostasis was well-controlled, resulting in minimal swelling and bruising.
In reality, there is still more swelling around the glabella and nasal tip compared to the final result,
but as planned, a nearly straight line was achieved, the deviated nose was straightened,
and from the front, the glabella and nasal tip gained dimension, with the tip becoming slimmer.
Even though the alar base was not specifically touched, the flared appearance improved significantly.
2 Weeks Post-op

There wasn't much swelling at 1 week, but by week 2, it is clear that the swelling around the glabella and nasal tip has subsided significantly.
Usually, major swelling tends to go down around 10 days to 2 weeks post-op.
2 Months Post-op

Although there wasn't much swelling from the beginning,
looking at it serially, you can see that the swelling at the glabella and nasal tip from week 1 has largely resolved after two months.
When there was swelling, the nose looked slightly upturned from the front, but this improved a lot after two months.
As planned, a nearly straight line was achieved, the deviated nose was straightened,
and from the front, the glabella and nasal tip gained dimension, with the tip becoming slimmer.
Even though the alar base was not specifically touched, the flared appearance improved significantly,
and as the overall swelling subsided, it became more natural. When viewed from the side, there is almost no drooping of the nasal tip,
and the height is well maintained.

Yes, it can. This patient desired improvement for their crooked nose, and we used septal cartilage and donated rib cartilage to build a supporting column and straighten the crooked nose. Two months after surgery, they achieved the straight nose line as planned.

Yes, it can be improved. This patient had a round nasal tip and drooping nostrils, but by using ear cartilage at the nasal tip, we created a more slender look, and the nostrils were also improved, resulting in an overall立体 (three-dimensional) nose shape.
What materials can be used for rhinoplasty?
In this case, septal cartilage and donated rib cartilage were used to build the nasal column, and ear cartilage was used for the nasal tip. Silicone with a straight line feel was used for the nasal bridge to create the natural nose line the patient desired.
Can nostrils be improved without separate surgery?
Yes, it's possible. In this patient's case, even without separate nostril surgery, the flared appearance significantly improved after nasal tip surgery. The overall change in nose shape can naturally harmonize the nostrils as well.
