Today, we will review a men's eye surgery case.
Pre-operative Analysis





The patient is a male high school student who visited us with sleepy and stuffy-looking eyes.
When we actually measured his eye-opening strength, it was on the weaker side,
and he was heavily relying on his forehead muscles to open his eyes.
For a definitive ptosis correction, we planned an incisional double eyelid surgery combined with ptosis correction.
There was some excess skin on the upper eyelids, but it was not sagging,
and since there wasn't a lot of orbital fat, we decided to excise only a small amount of skin without fat removal.
The eye shape itself is horizontally long, and although a slight epicanthal fold is visible at the front,
the lacrimal caruncle is sufficiently exposed and has a pointed shape,
so we did not recommend epicanthoplasty or lateral canthoplasty.
Consultation and Surgical Process

We could have tried a non-incisional method + ptosis correction,
but we anticipated an underwhelming result where the eyes would only become slightly clearer,
so we recommended the incisional method + ptosis correction.
He wanted a natural line, preferring an inner double eyelid rather than a completely monolid look,
so we designed a low in-line to prevent the results from looking overly burdensome.
The eye-opening strength was measured to be weaker on the right side.
If you look at the pre-op photos, there seems to be no difference between both sides when looking straight ahead,
but when he opens his eyes wide with force, you can see the right eyebrow and forehead lifting much more.
This is a compensatory action due to the weak eye-opening strength,
therefore, we needed to apply a greater degree of ptosis correction on the right side.
Other asymmetries were not significant, so we designed the line height and skin excision amount to be the same on both sides.
After making the incision and dissecting, we confirmed that there wasn't much upper eyelid fat.
After securing the ptosis correction points on both sides as planned,
we had the patient open his eyes during surgery to check if the exposure of the black pupils was similar.
We also checked if the inner and middle sections connected naturally,
and if necessary, we released and re-secured the fixation points to ensure symmetry.
Afterward, we created the double eyelid line at an appropriate depth,
checked again to ensure the line extended well,
and carefully completed the skin suturing to prevent any depressed scarring.
Post-op Day 7





The stitches were removed on the 7th day after surgery. There is some minor bruising and swelling,
but considering that rhinoplasty was performed together, it is not severe.
The patient mentioned that on the morning of the visit, the right eye was swollen and the end of the line seemed to overlap slightly,
but in most cases, as the swelling subsides, the line will not overlap and will extend smoothly as a single crease.
He said that opening his eyes has become much more comfortable, and he no longer uses his forehead, so his eyebrows do not lift either.
Post-op Week 2





In the photos taken at 2 weeks, the bruising has completely faded, and the line has become more natural.
The line on the right eye is settling in well without any overlapping.
For incisional double eyelid surgery, we explain that the swelling will continue to subside daily and settle for about two months,
and we check the primary results around the two-month mark.
Post-op Month 2





Looking at the 2-month photos (45-degree view), the patient had an eyebrow tattoo done on the same day,
so there is slight swelling, but the inner double eyelid has settled naturally without the end of the line widening.
The ptosis correction was also definitively achieved, providing not only clarity to the eyes
but also eliminating any wrinkles on the forehead.
When the eyes are closed, no depressed scars are visible,
and over time, it will fade into just a faint line.
Despite using the incisional method, the scarring is not a significant burden.

Is incisional blepharoplasty always necessary for male ptosis correction?
It depends on the patient's eye condition. Non-incisional ptosis correction is possible, but if the degree of eye clarity is expected to be unsatisfactory or if the strength of opening the eyes is very weak, incisional ptosis correction is recommended for a definite effect.
When are the stitches removed after surgery?
Usually, stitches are removed on the 7th day after surgery. At the time of stitch removal, some bruising and swelling may remain, but by the 2nd week, most of the bruising will have subsided, and the double eyelid line will begin to settle more naturally.
How is surgery performed if the strength of opening both eyes is different?
The degree of ptosis correction on both sides is adjusted differently to achieve symmetry. During surgery, the eyes are opened to check if the exposure of the black pupil is similar, and if necessary, the fixation points are readjusted to precisely correct the asymmetry of both eyes.
Will severe scars remain after incisional double eyelid surgery?
No, over time, only a faint line will remain. During surgery, skin suturing is meticulously completed to prevent depressed scars, and by the 2nd month, the scar becomes natural enough not to be a significant burden even when the eyes are closed.
Can male double eyelid surgery be done without being too noticeable?
Yes, it is possible. If you prefer an in-line double eyelid rather than no double eyelid at all, the line can be designed as a low in-line, completing a natural and clear eye shape without looking too obviously surgical.
