A female patient in her 30s visited our clinic wanting to change her unnatural double eyelids into a more natural look.

She had incisional double eyelid surgery 5 years ago,

but the fold loosened after a year, so she underwent a second incisional double eyelid surgery along with ptosis correction.

Currently, her eyelashes are everted, the eyelid mucosa is highly visible, and the deep double eyelid fold gives her a fierce appearance.

Additionally, the front line is high and appears sharply angled, creating an uneven and unnatural look rather than a smooth curve.

Before Surgery

The patient has medium-thickness eyelid skin, and while both double eyelids are not actually extremely high,

they are pulled deeply inward.

In particular, the front part of the left eye has a thicker, sausage-like line.

The everted eyelashes and exposed mucosa contribute to a fiercer impression.

The double eyelid line is also sharply angled, making it look unnatural.

She wanted to eliminate the sausage-like appearance and correct the everted eyelashes

to achieve a soft and natural double eyelid.

Surgical Plan

Although the patient had undergone two previous eye surgeries and had prominent scarring,

she did not lack so much skin that her eyes couldn't close fully.

Furthermore, she did not have a deficiency of fat tissue in her upper eyelids,

making her eyelid condition suitable for revision surgery.

We planned a revision double eyelid surgery to correct the sausage appearance and lower the line.

Since there was almost no excess eyelid skin available,

we planned to minimize skin excision to within 1mm.

Additionally, by utilizing the remaining fat in the upper eyelid,

we corrected the previously depressed scar areas.

7 Days Post-Op

There is still some swelling in the upper eyelids, but compared to before the surgery, the eyelashes have come down,

and the mucosa is noticeably less visible.

You can see that the double eyelid line has taken on a smooth, curved shape.

The final change in the depth of the double eyelid will be visible once the swelling subsides further.

3 Months Post-Op

This is the appearance 3 months after the surgery.

You can see that the depth of the double eyelid has improved compared to before.

The fierce and sharp-looking eyes have transformed into a soft and natural shape.

Undergoing multiple upper double eyelid revision surgeries does not necessarily mean you will have more scarring

or a stronger impression.

Depending on the condition of your eyelids, if there is a certain amount of skin and fat tissue remaining,

it is possible to perform a surgery that changes thick and deep double eyelids back into natural ones.

If I undergo multiple eye revision surgeries, will my scars definitely worsen and my impression become fierce?

No, that is not necessarily the case. Depending on the condition of your eyelids, if there is enough excess skin and fat tissue remaining, it is possible to improve to a natural eye shape. Repeated surgeries do not necessarily lead to more scars or a fierce impression; thick and deep double eyelids can be softened through an appropriate surgical plan.

Can the double eyelid line be lowered even if there is insufficient eyelid skin?

Yes, it is possible. Even with very little excess skin, the line can be lowered and sausage eyes can be corrected by minimizing skin excision to less than 1mm. However, the exact possibility of surgery will be determined by a specialist after directly examining the patient's eyelid skin and scar condition.

Can symptoms like upturned eyelashes and exposed conjunctiva after double eyelid surgery be corrected?

Yes, they can be corrected. If the existing double eyelid is pulled deeply, causing eyelashes to turn up and the conjunctiva to be exposed, revision surgery can release the pulled tissue and lower the line. As the swelling subsides after surgery, the eyelashes gradually return to their normal position and conjunctival exposure decreases, resulting in a softer impression.

How are sunken scars from previous surgeries treated?

They are corrected by utilizing the remaining fat tissue in the upper eyelid. During eye revision surgery, by redistributing or filling the patient's own eyelid fat into the deeply sunken scar areas, the scars can be alleviated and the double eyelid line can be made smoother.

When can I see the final results after eye revision surgery?

Generally, you can see the final results about 3 months after surgery, when most of the swelling has subsided. Improvements such as upturned eyelashes can be confirmed as early as 7 days post-surgery, but changes in the depth of the double eyelid and the natural curve of the line can be most accurately judged after 3 months, when residual swelling has sufficiently gone down.
