Today, I would like to review a case of a male patient who was concerned about his sagging double chin,

focusing on his double chin liposuction & platysmaplasty (muscle tie) procedure.

Today, I would like to review a case of a male patient who was concerned about his sagging double chin, focusing on his double chin liposuction & platysmaplasty (muscle tie) procedure.

This patient is a male in his late 20s with a short chin and a severe double chin.

In fact, for those with a small lower jaw, performing a double chin muscle tie to tighten the platysma muscle is necessary

to bring out the cervicomental angle (chin-neck angle), making this an ideal situation for surgery.

We also planned to perform double chin liposuction together, not only on the front of the chin (the so-called double chin area) but also

along the overall jawline extending near the ears, to create a sharper appearance.

Consultation and Surgical Method

The surgical process for double chin liposuction and muscle tie is as follows:

First, we suction the fat under the chin through a small incision of about 2mm under the earlobe.

Sufficient fat must be suctioned to achieve both a volume reduction effect + an effect where the skin adheres more tightly.

After primarily resolving the fat above the platysma muscle using this method,

we make an incision of about 2cm in the crease under the chin to proceed with the muscle tie surgery.

We dissect the remaining adhesions between the platysma muscle and the skin to allow the platysma muscle to move freely,

and then partially remove a narrow strip of the central muscle.

This exposes the subplatysmal fat (fat under the muscle). In this patient's case, the amount of this fat was also significant,

so we removed it sufficiently to reduce the volume.

Afterward, we additionally tied the digastric muscle located deeper

to maximize the tightening effect.

Following this, the surgery concludes by tying the platysma muscle with appropriate tension as originally planned.

1 Week Post-Op

After performing double chin liposuction and muscle tie, proper compression is necessary to prevent blood from pooling.

Immediately after, both taping and a facial compression garment are worn, and we advise wearing them constantly, especially for the first 48 hours.

After that, the taping is maintained until the stitches are removed at 1 week, but we tell patients they can remove the compression garment when going out.

At 1 week, there is no possibility of additional bleeding, but since minor swelling can last a long time due to gravity,

we explain that it is good to wear the compression garment continuously when at home, recommending this for up to 3-4 weeks.

Looking at the photos taken after removing the taping on the 7th day post-op,

you can see that the cervicomental angle, which was previously unnoticeable, is now properly visible, especially in the 90-degree view.
