Today, I would like to review a fat grafting case of a male patient who was concerned about multiple eyelid folds.

Before Surgery

Before visiting our clinic, this patient had double eyelid surgery and

upper eyelid fat grafting performed only on the left eye.

However, the multiple folds on the left eye did not disappear even immediately after the surgery,

and he was complaining of discomfort when opening and closing his eyes.

Multiple double eyelid folds not only cause discomfort in daily life,

but aesthetically, they can also give a sleepy impression.

Diagnosis and Surgical Method

In the patient's case, when he first visited,

it was determined that there was a lack of fat in the left upper eyelid,

so we decided to harvest fat from the inner thigh and perform

micro-fat grafting on the left upper eyelid.

The surgical method involves making an incision to dissect the orbital septum and injecting micro-fat into the septum.

Compared to the non-incisional method of just making a puncture, the incisional method has fewer issues such as lumpiness or poor fat engraftment,

which gives it the advantage of allowing a larger volume of fat to be grafted.

During the follow-up visit 2 years later, the multiple folds on the left eye from the first surgery had engrafted well

and were maintained, but over time, the fat on the right side had also become significantly sunken, so we performed

micro-fat grafting on the right side using the same method.

After 2nd Surgery (Final)

There is still some swelling, but the sunken areas have been well filled,

the discomfort when opening and closing the eyes has been greatly improved,

and the dull and tired-looking impression also seems to have improved significantly.
