Incisional Double Eyelid (Revision) + Ptosis Correction + Under-Eye Fat Repositioning

This patient had incisional double eyelid surgery, ptosis correction, and epicanthoplasty 12 years ago.
Over time, the double eyelid folds loosened, and multiple creases formed on both eyes,
causing the double eyelid lines to become asymmetrical.
As the asymmetry progressively worsened, she was attending consultations to get revision surgery.
Pre-operative Analysis


Patient's Condition Before Surgery

Due to the multiple creases on both the left and right eyes, the lines were uneven,
giving an aged, tired, and stuffy impression with an unnatural eye shape.
Desired Eye Shape
The patient wanted the overall double eyelid height to be a line slightly higher than the left eye,
and hoped to correct the asymmetry and multiple creases for a harmonious look on both sides.
She also wanted to improve the dull impression caused by the bulging under-eye fat.
Diagnosis and Surgery
✅ Semi-out line double eyelid + Ptosis correction
✅ Under-eye fat repositioning
The multiple creases on the upper eyelids were improved with incisional double eyelid surgery and ptosis correction.
Although the patient wanted a semi-out line for the double eyelid,
creating a semi-out line would make the front line too thick, so we decided to create a clear in-out line
that is higher than the previous line.
For the under-eye area, the overall bulging fat and dull appearance
were to be improved through under-eye fat repositioning.
Post-operative Progress


2 Weeks After Surgery
This is the appearance after stitch removal at 2 weeks post-surgery.
There is not much swelling, and the double eyelid lines on both sides are beautifully symmetrical.


2 Months After Surgery
✅ Clear in-out line achieved
✅ Dull under-eye area improved
Through incision + ptosis correction, the multiple creases disappeared, and the double eyelid lines became clear,
showing significant improvement.
Additionally, by performing under-eye fat repositioning together,
the bulging under-eyes and the shadowed, tired impression were improved.
As the under-eye area became flatter overall,
the patient achieved a younger and brighter appearance.
