Hello,

💚your beauty universe! I am Dr. Shim Jung-hwan, head director of Uber Plastic Surgery :)💚

Today, in part 3 of our revision rhinoplasty series, we have prepared

information on the essential factors you must consider before undergoing revision rhinoplasty.💚

Revision Rhinoplasty: The Clinic from the First Surgery VS A Different Clinic?

If the nasal strut is sturdy and the areas of dissatisfaction are not major, it might be a good idea to visit the clinic where you had your first surgery.

In fact, during consultations, if it seems like only a minor reinforcement is needed,

I sometimes recommend visiting the original clinic to get a revision.

Among my own patients, there are many cases where I perform minor revisions for fine adjustments.

Assuming there are no major issues, it is often best to have the procedure done by the surgeon who knows exactly how the previous surgery was performed.

However, if the nose has drooped significantly or if a deviated nose was not properly corrected,

it would not be a bad idea to visit multiple other clinics as well as the original one to hear the opinions of various surgeons.

When is the best time for revision rhinoplasty? After 6 months?

Generally, for all surgeries, it is best to perform a revision after 6 months.

It takes about this long for the internal scar tissue to settle and soften, so we set this period as a minimum

and recommend undergoing surgery after that.

What do patients consider most important during revision rhinoplasty?

I believe patients consider it most important to definitively correct the parts that were not resolved in the previous surgery.

If the line is not what they wanted or doesn't suit them, it is necessary to have a long and clear design consultation.

If the nasal tip has drooped, it will be important to maintain it firmly so it doesn't droop again,

and if the nose is deviated, it will be important to properly correct the deviation during the surgery.

Is there a trend in revision rhinoplasty?

There doesn't seem to be a specific trend for revision surgeries.

Since the patient has already undergone surgery once, if they decided on a revision due to the shape,

it is important to clearly consult and communicate regarding what they want versus what suits them,

as well as what is possible and what is not.

For revision surgery, which do you recommend more: donated rib cartilage vs. autologous rib cartilage?

For the first surgery, since the septal cartilage is still intact, I often recommend supplementing any lacking septal cartilage with donated rib cartilage.

However, for revision surgery, if I feel the nose and check the CT scan and expect that a lot of cartilage has been absorbed,

or if the previous strut was weak and collapsed, or if the patient wants to raise the height further after their first surgery,

I recommend autologous rib cartilage, which is sturdier and provides a more sufficient amount.

What should be taken care of more carefully after a revision surgery compared to the first surgery?

Since the surgery time is relatively longer and there can be more bleeding, swelling may occur more easily,

so it would be good to apply ice packs diligently for 3 days after the surgery.

Just because it is a revision surgery doesn't mean there are special precautions or aftercare required.

You can manage it the same way you did after your first surgery.

Is there a way to find a skilled specialist to avoid needing another revision?

Since there are many aspects you can only know by verifying their surgical work and actually undergoing surgery with them,

I think it won't be easy for patients to find one.

It is important to look up many reviews and find a surgeon who excels at the shape you desire. It is also good to observe how much attention they pay to each individual patient during the consultation. Because every patient wants something different, poor communication in this area can lead to another revision, so you should check if the surgeon seems capable of reflecting your needs through a thorough consultation.

What should I prepare to communicate well with the surgeon?

Since there can be limits to what a patient can express in words,

I recommend bringing reference photos of your ideal look.

Because every surgeon and patient has different standards for what is considered glamorous or natural,

it is best to bring photos to make things clear.

What kind of photos should I prepare?

Other people's review photos are fine, and photos of celebrities are fine too.

Many people also find photos on Instagram. Since the goal is to understand what you want, the more photos you have, the more helpful it is.

Frequently Asked Questions

When is the best time to get revision rhinoplasty?

It is generally recommended to perform revision rhinoplasty at least 6 months after the initial surgery. This is because it takes a minimum of 6 months for the internal scar tissue in the nose to settle and soften. Surgery after this period allows for safer and more accurate correction.

Should I go back to the same hospital where I had my first rhinoplasty for revision surgery?

If the dissatisfaction is minor and requires only subtle correction, it is advisable to visit the hospital where you had your first surgery. They will have the best understanding of your previous surgical state. However, if the issues are severe, such as significant drooping or poorly corrected crookedness, it is recommended to consult with several other hospitals as well.

Which cartilage is better for revision rhinoplasty: donor rib cartilage or autologous rib cartilage?

For revision surgery, autologous rib cartilage is more recommended due to its sufficient quantity and strength. Revision cases often involve significant cartilage absorption or weakened existing structures leading to a dropped nasal tip, and many patients desire a higher nose, making the strong support of autologous rib cartilage suitable.

Are there any special post-operative care instructions to follow after revision rhinoplasty?

Thorough ice packing for the first 3 days after surgery is the most important. Revision surgery typically takes longer than the initial surgery and may involve more bleeding, which can lead to increased swelling. Other than that, you can manage it in the same way as your first rhinoplasty.

What should I prepare when consulting with the doctor for revision rhinoplasty?

It is best to prepare as many 'wannabe' photos of your desired nose shape as possible. Since everyone has different standards for 'glamorous' and 'natural' looks, showing visual materials such as celebrity photos or post-op review photos will greatly help the medical staff accurately understand your needs.
