Hello~ Your beauty universe!

Q: What is the difference between under-eye fat repositioning and under-eye fat removal?

Under-eye fat repositioning refers to a surgery where the tear trough ligament is released using the under-eye fat

to create a flat surface underneath.

If there is a lot of bulging under-eye fat and the tear trough ligament is not severe,

some people undergo surgery by only removing the fat

without touching the ligament.

However, in reality, there are not many cases where simple fat removal is sufficient.

Because under-eye fat repositioning structurally makes the under-eye area firmer and more stable,

in most cases, repositioning provides more satisfying results than removal.

💡When is under-eye fat repositioning needed?

Under-eye fat repositioning is effective in the following cases:

1. When the under-eye fat is bulging

2. When the area below the under-eye fat, the tear trough ligament area, is sunken

3. When two lines of shading (shadows) are visible below the aegyo-sal (under-eye roll)

In this case, if the tear trough ligament is released and the remaining fat is repositioned,

both the bulging and sunken areas are improved together,

making the under-eye area naturally flat.

📌Case 1

This is a case where the fat bulged below the aegyo-sal,

and the tear trough ligament area below it was sunken, showing two lines of shading.

For the bulging fat, we performed under-eye fat repositioning surgery

by releasing the tear trough ligament and repositioning it to make the under-eye flat.

Looking at the overall appearance, you can see that the under-eye area,

which used to look hollow and tired, has become flat.

📌Case 2

This is a type where the aegyo-sal is slightly small and the tear trough ligament area is severely sunken,

with an overall lack of fat volume.

To fill the sunken tear trough ligament area with under-eye fat

without making it look bumpy,

we performed a surgery to fill the under-eye area with small-particle thigh fat.

At week 1, before the fat was fully engrafted,

some swelling was visible,

but by weeks 2 to 3, the previously sunken area

became flat and gained volume, bringing dimension and vitality to the face.

📌Case 3

This is a case where the under-eye fat was bulging while the tear trough was simultaneously sunken.

Although the fat was slightly bulging overall,

without removing the fat, simply releasing the tear trough ligament and repositioning

the remaining fat was enough to achieve a sufficiently flat and natural result.

📌Case 4

This is a case where under-eye fat repositioning and under-eye fat removal were performed together.

Due to left-right asymmetry, the fat on one side was bulging more,

so a small amount of fat was removed on one side, and the rest was repositioned.

After the surgery, the under-eye shading was reduced from two lines to one,

making it look much cleaner.

If you are concerned about your under-eye area looking both bulging and sunken at the same time,

giving you a tired or hollow impression,

you can improve it into a flatter and more vibrant eye shape through under-eye fat repositioning.

Since the necessary surgical method varies depending on individual conditions,

it is best to make a decision after an accurate diagnosis and consultation at the clinic.

Frequently Asked Questions

What is the difference between under-eye fat repositioning and under-eye fat removal?

Under-eye fat repositioning is a surgery that opens the under-eye ligament and spreads the fat evenly downwards, while under-eye fat removal is a surgery that simply removes only the protruding fat. Structurally, it makes the under-eye area firmer and more stable, so in most cases, repositioning yields more satisfactory results than removal.

Who needs under-eye fat repositioning surgery?

It is effective for those who have protruding under-eye fat and a sunken under-eye ligament area. Especially if two lines of shadows or darkness appear below the aegyo-sal (under-eye fat roll), giving a tired and haggard impression, the remaining fat can be repositioned to naturally and smoothly improve the under-eye area.

Is surgery possible even if the under-eye fat volume is insufficient and severely sunken?

Yes, it is possible. If the under-eye ligament is severely sunken and the overall fat volume is insufficient, under-eye fat repositioning can be combined with micro-particle thigh fat grafting to fill the area. This can flatten the sunken areas and add volume, restoring dimension and vitality.

When does the swelling subside after under-eye fat repositioning surgery?

Typically, swelling subsides and the area becomes natural around 2 to 3 weeks after surgery. Up to 1 week after surgery, some swelling may remain before the fat settles, but over time, you can observe the sunken areas flattening and gaining volume.

How is surgery performed if the degree of protrusion of under-eye fat differs on both sides?

In cases of asymmetry, under-eye fat repositioning and under-eye fat removal are performed concurrently. A customized approach is applied where a small amount of fat is removed from the side with more protrusion, and the rest is repositioned, effectively improving under-eye shadows.
