Is looking older solely due to facial sagging?

There are exactly three main factors.

✔️Sagging ✔️Wrinkles ✔️Volume Loss

When people think about looking older, most only consider lifting procedures~

It is true that lifting is the key point.

However, as a board-certified plastic surgeon,

I would like to talk about volume loss...🙆‍♂️

Why does facial volume loss make you look older?

If you put photos of someone in their 20s and 40s side by side and compare them,

✔️The temples become hollow,

✔️The side cheeks next to the cheekbones sink in,

✔️The upper eyelids lose volume,

✔️And the under-eye area also becomes slightly hollow.

These areas of volume loss make you look very tired

and give off an older impression.

Looking at the biggest difference between young children and older individuals,

how does the face change as we age?

The skull-like shape, meaning the underlying bone contour,

becomes much more prominent.

When you look at a child's face, can you see their bone contours?

You can't, right?

Filling in those hollowed areas

is, in my opinion, just as incredibly important as lifting.

How can we resolve facial volume loss?

The solution is actually

to fill in the sunken areas.

There are largely three methods

for adding volume to those hollow areas.

① Fillers

② Fat Grafting

③ Collagen Boosters

So, which areas are best suited for fillers, fat grafting, or collagen boosters?

#case 1

This is a patient whose forehead to temples is slightly bumpy and hollow.

Actually, in cases like this, I tend to recommend fat grafting more.

The first reason is simply because a large volume is required.

Even if you try to fill the forehead and temples entirely with filler, you would need about 10cc or more.

Because the physical properties of fillers are very soft,

if you inject 10cc of filler into the forehead like this,

the tissue won't hold it firmly in place.

It can migrate down, and when pressed, it might feel like slime

sinking right in.

I recommend fillers only for localized parts of the temples and forehead,

and if you want to resolve overall volume loss more comprehensively,

fat grafting is the better option.

Therefore, for the model above,

we performed fat grafting to address the overall hollowness of the forehead and temples.

#case 2

This other model also had fat grafting on the forehead and temples,

and you can see the before-and-after difference where she looks much younger and her face appears more beautifully rounded, right?

#case 3

Now, this is a case where the forehead and temples were treated with filler.

Earlier, I mentioned that when filling the entire hollow areas of the forehead and temples,

I recommend fat grafting more,

but for this patient, the upper forehead was already quite prominent like a dome,

so it didn't seem like a large amount of volume was needed.

Since even a slight fill could yield a great effect,

we only added a small amount of filler.

As the volume was restored, her narrow forehead appeared wider,

and her overall image became much brighter.

Therefore, in cases like this,

I believe you can see excellent results with fillers as well.

#case 4

For this patient, she stated she didn't want her forehead to protrude any further,

and due to her temples and cheekbones,

her overall face shape had a sunken, hourglass-like contour.

This made her face look longer and gave off a tired impression.

Here is the result after treating it with temple fillers.

It looks much softer than before the procedure, right?

Her face also looks somewhat younger.

As we age,

it becomes more common for the side cheeks to sink in.

Some people have sunken side cheeks because their cheekbones are large,

and for those individuals, it would be best to reduce the cheekbones while filling the side cheeks.

But even in other cases, adding volume to the temples

can give the face a much softer appearance.

#case 5

This is a case where the side cheeks were filled with a collagen booster.

Actually, I tend to prefer collagen boosters for filling the side cheeks.

The reason is that when filler is injected, it doesn't lift well in this area.

For example, the base needs to be firm

in order to push the volume up,

but because the base here isn't firm and doesn't provide support,

it has the characteristic of not lifting well even if you want it to.

Because of this, if you get fillers here,

it might look fine at first but then sink back in,

leaving you a bit disappointed later on.

However, since collagen boosters

stimulate the tissue to produce collagen itself,

collagen gradually builds up over time,

creating natural volume in the face, which is why I prefer them.

Then what about fat grafting?

Sometimes the skin is too tight,

making it difficult for the fat to engraft well.

In fact, the side cheeks are the area where fat engraftment is the poorest.

Because the tissue is so tight, even when fat is injected,

it repeatedly fails to engraft and sinks back in...

In such cases, I tend to recommend collagen boosters.

#case 7

This is a case where the sunken side cheeks

were filled with a small amount of filler.

Even though we only injected a little bit,

the added volume made the face look much softer than before.

In conclusion, if a large amount is needed in one area,

I tend to avoid using fillers,

but if only a small amount is needed, fillers actually offer the best cost-effectiveness.

This is because they are affordable, and if you really don't like the result later, they can be dissolved and removed.

Depending on the area, there are cases where collagen boosters are better,

and cases where fillers are better.

What are the types of fillers and collagen boosters?

Foreign filler products include

Juvederm and Restylane.

Among domestic fillers, there are Chaeum and Neuramis.

✅Juvederm

✅Restylane

✅Chaeum

✅Neuramis

You can think of these as products that

completely dissolve and disappear over time.

Collagen boosters can be broadly divided into two types.

✅Juvelook

✅Olidia

These are PLLA products that stimulate collagen production,

but because the amount of collagen produced is slightly lower,

they are products that require multiple treatment sessions.

✅Ellanse

✅Lafullen

These are also collagen boosters,

but they produce so much collagen that they are known as semi-permanent fillers.

The originally injected materials disappear in about 2 to 3 years,

but the newly generated collagen remains for a long time.

You can think of them as collagen boosters that yield a much larger amount of collagen.

However, there are also disadvantages.

They are a bit more tricky to handle than fillers and offer better effects,

but if side effects occur, issues like granulomas or nodules can form.

Because removal is also difficult,

many people feel afraid and avoid them before trying.

That is why I think this is an area where plastic surgeons can perform even better

than with standard fillers :)
