Today, we will be reviewing double chin liposuction and double chin thread lifting.

First, let's analyze the problem through the patient's pre-operative photos.

Pre-operative Analysis

Although the patient does not have a lot of fat overall,
there is some fat around the double chin area and below the jawline,
and slight skin sagging is visible, particularly in the double chin area.
Consultation and Surgical Process

Many people are concerned about their double chin, but among the various solutions,
I believe deciding which method is best is the most difficult and important part.
We must differentiate between necessary and unnecessary procedures by considering various factors,
such as whether the jawbone is small, the amount of fat, and the degree of skin sagging.
For this patient, we decided to perform double chin liposuction and double chin thread lifting.
There were many patients, but the reason I am doing a case review with this specific patient is
that they perfectly meet the baseline criteria I use when deciding on double chin surgical techniques:
1) If the skin were tighter than this patient's, thread lifting would not be highly recommended.
2) If the fat volume were less than this patient's, the satisfaction with liposuction might be lacking.
3) If the cervicomental angle were larger than this patient's, a muscle tie (platysmaplasty) would be effective.
Of course, performing a muscle tie is the most powerful in terms of effectiveness,
but because it leaves a scar under the chin, I only recommend it to those who absolutely need it.
For this patient as well, I believed the benefits gained compared to the scarring were not significant,
so a muscle tie was not performed.
The patient also mentioned they wanted their jawline to become more defined.
Since double chin liposuction is not limited to just the double chin at the tip of the chin,
but is performed across the entire boundary between the jaw and neck, you can see effects along the entire jawline.
The surgical process involves making 2-3mm incisions at the ends of both earlobes and the tip of the chin,
injecting a solution to soften the fat, waiting for a certain period, and then proceeding with the liposuction.
After the liposuction is finished, thread lifting is performed by inserting one thread each from two locations in the exact center of the chin,
directed towards the back of both ears.
The three points where liposuction was performed have 1-2 sutures each,
and the dressing is finished with compression taping.
1 Week Post-Op

If only thread lifting were performed, a compression dressing under the chin would not be applied.
However, in the case of liposuction, it needs to be pressed well for about the first week so that bodily fluids do not accumulate and are absorbed well,
which plays a role in reducing overall swelling.
Looking at the one-week photo, the jawline has become distinct,
and you can see that the cervicomental angle has improved as the double chin area has gone in.
Especially in the early stages, the threads hold strongly.
These threads need about 2 weeks to settle into place,
and during that time, you should avoid actions that pull the neck area (like stretching) as much as possible
so the threads loosen less and the effect is maintained for a long time.
1.5 Months Post-Op

Even in the photo taken about a month and a half later, the cervicomental angle is well maintained,
and the reduced volume under the chin is also holding up well.
The scar under the ear is at its reddest and most raised during this period,
but after about 6 months, it will fade significantly and become barely noticeable.
