often make revision surgery necessary.
Today, we will look at a patient who underwent surgery at our clinic due to multiple folds, asymmetry, and scarring,
along with photos of the actual case!
Incisional Double Eyelid Revision & Fat Transposition

A female patient in her 20s visited our clinic with her mother.
She had undergone incisional double eyelid surgery, ptosis correction, and epicanthoplasty 4 years ago,
and reported that about 8 months post-surgery, the right fold loosened and multiple folds appeared.
Since then, the asymmetry progressively worsened, and the scars became highly visible, leading her to consult with several clinics for revision surgery.
The reason she decided to have her surgery at our clinic was that she received the most accurate diagnosis of her current condition here,
and she felt she could trust us because we only recommended necessary procedures without any unnecessary overtreatment.
Pre-operative Analysis


Patient's Pre-operative Condition
Right eye: The double eyelid fold has loosened, and multiple folds have formed above it.
Left eye: The eyelashes are everted (lifted), and the double eyelid fold is dark and deep.
She mentioned feeling discomfort due to a pulling sensation in the skin below the double eyelid when opening her eyes, and the skin below the fold on the inner side is wrinkled, looking unnatural.
She stated that the swelling was severe during her previous surgery, and she was unable to close her eyes fully for several months post-op.
Even now, she has a slight lagophthalmos (incomplete eye closure) in her right eye, the scars on both double eyelids have widened,
and there are depressed scars extending outward from the ends of both lines. Her right eyebrow is also elevated compared to the left.
The depressed scar in the middle of the left eye is severe and highly visible when looking down.
What kind of eyes did the patient want before surgery?
The patient wanted to eliminate the current asymmetry and achieve natural-looking double eyelids.
She also wanted her eyes to open and close smoothly without any feeling of discomfort.
Furthermore, she hoped to improve the depressed scars on the double eyelid lines and resolve the eyelash eversion and skin pulling sensations.
Diagnosis and Surgery
This patient has thin skin and slightly protruding eyes.
Additionally, there is almost no excess skin or fat tissue remaining.
Therefore, we planned to eliminate the multiple folds by utilizing the remaining fat inside the eyelids as much as possible, without excising any skin.
We planned to release the adhesions caused by the previous ptosis correction and resolve the eyelash eversion and pulled skin.
We determined that additional canthoplasty was unnecessary, and the epicanthoplasty did not require reconstructive surgery.
※ As a result, we performed an incisional double eyelid revision and eyelid fat transposition.
Post-operative Progress

This is the appearance after stitch removal on the 5th day post-op.
Although some swelling remains, both double eyelid lines are well-formed symmetrically, and the multiple folds have disappeared.
There are no signs of incomplete eye closure, and the depth of the double eyelids is not too deep, looking appropriately defined.


This is the appearance at 6 weeks post-op. The swelling has almost completely subsided.
The size of both double eyelids has been shaped into the soft, slightly visible lines that the patient desired.
The depressed scars visible when looking down have disappeared, along with the skin pulling sensation and eyelash eversion.
The patient also mentions that opening and closing her eyes has become very smooth and comfortable.
Although we need to monitor the final results up to 6 months post-op, the patient is currently very satisfied,
reports that her eyes feel comfortable, and the aesthetic results are excellent.
