What are the reasons people think their first eye surgery has failed?

Surgery naturally requires not only money but also a lot of time and effort when you do it once.

You have to wait at least 6 months for the swelling to go down and for it to look natural,

so it is best to get the surgery right the first time.

However, there are cases where you inevitably have to consider revision surgery.

The most common cases are when the double eyelid fold has loosened, or when the double eyelid did not turn out pretty.

Not turning out pretty can mean asymmetry, a thick line, an unnatural shape, and so on,

and some people consider revision surgery not just because of the shape, but because their eyes feel too uncomfortable.

What is the reason double eyelids loosen?

Just as everyone's eyes look different, the reasons for a loosened double eyelid are also very diverse.

A non-incisional (suture method) double eyelid can easily loosen if the fold is too small, and it can also loosen if it is too large.

With the incisional method, it can loosen if there is a lot of fat and thick skin remaining,

or it can loosen as the fixation strength weakens.

If the upper eyelid is thin and sunken, the double eyelid may look loose and easily unravel, and it can also loosen if the eye-opening muscle is weak.

There are cases where it loosens even with the incisional method. Why does this happen?

For those who had surgery using the incisional method, it is highly likely they chose it because a non-incisional method wouldn't work well,

or because their eyes were prone to loosening in the first place.

There are cases where it loosens simply because the surgical fixation was weak and the adhesion did not form well,

but there are also cases where it loosens due to the structural nature of the eyelid, such as lacking volume in the upper eyelid or having weak eye-opening strength.

In the case of asymmetry, is it because the surgery was done incorrectly?

We all know that our two eyes are not exactly the same.

However, many people are unaware of this before surgery.

Even after surgery, the shape of the eyes, the surrounding bones, and the eyeballs haven't changed, so the difference between the two eyes remains.

However, if the size of the double eyelid visible when the eyes are open is similar on both sides, it won't look like severe asymmetry.

Therefore, during surgery, rather than trying to change the original shape of the eyes,

focusing on matching the size of the double eyelid when the eyes are open can greatly reduce the chances of needing revision surgery for asymmetry.

Regardless of whether the first surgery was natural adhesion or incisional, is revision surgery only possible through an incision?

Not all sausage double eyelids are the same.

Some people get a sausage shape simply because the line was set too high after a non-incisional surgery,

while others may get a sausage shape after an incisional surgery even if the line isn't that high,

because the fixation was made too deep and strong.

First, we need to find the cause of the current shape and then consider the surgical method.

If the first surgery was natural adhesion, it might be possible to lower the line using natural adhesion again after removing the buried sutures.

In what cases is revision surgery possible with natural adhesion?

As I mentioned earlier, if the first surgery was natural adhesion and suture removal is possible,

there are cases where surgery to lower the line with natural adhesion is possible after removing the existing buried sutures.

However, if adhesions have already formed and a crease remains even after removing the sutures, an incisional method might be necessary,

so the decision must be made after carefully checking the current condition of the eyelids.

Are there cases where natural adhesion is possible for revision even if the first surgery was incisional?

That is possible if the double eyelid has loosened to some extent,

but we don't usually call it a sausage eye when it's in a loosened state.

Yes, there was a female patient in her 20s who had surgery three times using the incisional method, but it kept loosening.

Looking at her eyelids, I could see scars from repeatedly getting surgery at a high position that didn't suit her eyelids.

And she said she had surgery three times from the same doctor.

Because she didn't change clinics, she wasn't in a state where an unnecessary amount of skin or fat had been removed.

For this patient, we were able to perform the surgery again using the non-incisional method by creating a lower double eyelid line that easily forms on her eyelids.

I heard that lowering the line is the most difficult among eye revision surgeries. What is the reason?

Because you have to eliminate the high double eyelid while creating a lower one.

It's not difficult if you lower the line by excising skin, but among those who have a large line,

many lack excess skin, making it difficult to excise even 1mm.

Then, you have to leave the high double eyelid site as is and create a new, lower double eyelid beneath it,

and it seems that saying exists because it is not easy to make it look clean.
