Following our last 'Non-Implant Rhinoplasty Part 1',

let's begin Part 2!

First of all, non-implant rhinoplasty can be done just on the nasal tip, but it can also be done on the nasal bridge, right?

Both the bridge and the tip can be augmented using cartilage and your own dermis.

However, most people who want this non-implant rhinoplasty say the exact same thing.

'I don't really care about my nasal bridge. I just want my nasal tip to be slimmer.'

They visit the clinic saying, 'I just want to define the line a little bit more.'

This is because most people who want non-implant rhinoplasty

actually don't have a particularly low nasal bridge to begin with.

They already have some height to their bridge.

And their face doesn't look too flat, either.

Therefore, they come in thinking that just tweaking the nasal tip a little will make it look better.

But the problem arises here, which is

Cases that should absolutely not be touched

When the side profile looks great, but from the front, the nasal tip looks bulky,

they come in saying, 'It looks too short. It looks too long.'

If it looks fine from the side but the nasal tip appears too long or too short,

how should we change the position of the nasal tip?

A situation might arise where it needs to be changed like this.

Then how do we create this line?

The line I marked in red cannot actually be created.

When I say non-implant isn't possible, patients

often say, 'But there's autologous rib cartilage~'

Sometimes they ask, 'Since there is autologous rib cartilage,

please place it here to create the line.'

Patients might think it's doable,

but in reality, it's much harder than you'd think.

The reason it can't be done is that the part indicated by the blue arrow

needs to be made into a clean line

for the autologous rib cartilage to form a line like silicone, as shown in the picture above.

This length usually requires at least 3 to 4 cm for the nasal bridge,

and sometimes up to 5 cm, to be inserted all at once.

If it is inserted in broken pieces instead, a step-off (unevenness) will naturally occur, right?

This step-off becomes very noticeable when light hits it.

Therefore, to create a clean line of about 3 to 5 cm,

the autologous rib cartilage must be carved,

And when carving it, you can't just carve it roundly from the side;

when viewed from below, the nasal dorsum part must be rounded along this line.

It must be carved into a line like this.

Can it be carved like that?

If you try really hard, you might be able to carve it.

However, autologous rib cartilage has a phenomenon commonly known as warping.

When used as a whole block, it will eventually bend.

Because it will bend eventually, it will naturally result in a deviated nose.

That is why it is extremely difficult to do.

In fact, the cases that should avoid non-implant rhinoplasty are:

✔️Those with a good side profile

✔️Those who say, 'My side profile is fine, just touch up my nasal tip'

For these people, it is better not to use the non-implant method.

There are people who have a dorsal hump on their side profile like this.

For these individuals, non-implant rhinoplasty is a very good option.

This is because shaping the nasal tip while shaving down the hump like this

can result in a beautiful nose.

Ultimately, the amount being shaved down is much greater than the amount being raised or filled.

When there is something bumpy, shaving it down is much

easier to make it smooth.

If you need to build it up by adding more tissue like this, what should you attach it with?

You cannot attach it with something hard.

You have to attach it with something like clay,

but do we have anything like clay in our bodies? No, we don't.

Even if we do, it would be the dermis, but what happens to the dermis?

The disadvantage of the dermis is that it has an absorption rate.

Because of this absorption rate, even if you attach it, it will become bumpy again once it gets absorbed later.

However, people don't just look at your nose from the exact side profile;

everyone inevitably sees your nose from 180 degrees,

and since we take a lot of photos, in cases where a lot of shaving is required,

you can shave it down to make it smooth,

so the satisfaction rate is much higher when using the non-implant method.

On the other hand, cases with low satisfaction are humps like this.

For humps like this, patients sometimes come in saying, 'Please just fill in the sunken areas a little.'

From the side, it might look easy to fill.

But why are these difficult cases?

Assuming the nasal bridge is like this,

to fill it, it must be filled roundly like this.

But can it be filled like this?

You cannot carve cartilage and fit it perfectly like a Lego block.

That is why mismatches occur at these connection points.

Then, when strong light hits it and you take a picture, it becomes noticeable.

That is why people visit us for revision surgery.

Therefore, you must undergo the surgery clearly

knowing that it will not be perfectly smooth.

There are people whose nasal side profile is fine like this.

They ask, 'Please improve my protruding mouth.'

To eliminate a protruding mouth,

this angle needs to be sharp like a dagger. Then where would the nasal tip end up?

If we do it like this, it becomes an upturned nose, so that won't work.

The position of the nasal tip needs to come further forward relative to the mouth,

and the tip needs to reach the area I marked above.

Then the nasal tip becomes like this.

The nasal bridge also needs to be built up somehow by the shaded amount. It's the same thing.

Ultimately, the core of a good case for non-implant rhinoplasty is:

✔️When the amount of nasal bridge reduction and dorsal hump removal is large,

it is better to use the non-implant method.

✔️When there is a large amount of the nasal bridge to be filled, using the non-implant method actually has many disadvantages.

Then what is the standard for saying that?

Whether there is more to remove or more to fill is not simply determined by the current nasal bridge alone.

It must be considered based on when the nasal tip is moved.

Most people who opt for non-implant rhinoplasty only want to change their nasal tip,

and if the nasal tip is moved, the connection with the nasal bridge will inevitably not be smooth.

So you have to consider whether there is a lot to shave down to smooth out this uneven part,

or if there are many cases where you need to build it up to create the line.

That is how this conclusion is reached.

Therefore, when there is a lot to remove, non-implant rhinoplasty can yield very good results.

Using silicone itself wouldn't be very good in that case.

But when a lot needs to be filled, non-implant rhinoplasty

is not suitable.
